PET APPLICATION FORM

Please note the following rules regarding pets at Meadowridge:
- No pets allowed over 25 pounds – proof of pet weight is required at application
- Only domestic pets are allowed, including dogs, cats and small birds
- Cats must be indoor cats only with indoor litter boxes
- Birds must be indoor birds with cages; no birds allowed to live on lenai
- Proof of shots and neutering/spaying must be submitted with application
- Photo of pet must be submitted with application
