  •  Opt-In 2023 Post Hurricane Cleanup
    I/We understand and acknowledge that this shall remain in effect and that will be billed at
    the current Hurricane Rates as designated by the agreement signed and approved between the
    Bonita Lakes Board of Directors and clean-up vendor. The association’s agreement is here: DTE

    I/We understand and acknowledge that by Opt-In to this agreement the clean-up vendor
    will bill directly and that any and all disputes for services will be directed to the vendor directly.


     Opt-Out 2023 Post Hurricane Cleanup
    I/We understand and acknowledge that we are providing this Notice of Opt-Out for
    Hurricane Cleanup and by doing so will be responsible for all debris cleanup and removal post
    Hurricane for the 2023 Hurricane Season.
    I/We understand and acknowledge that by the above Opt-Out that any damages to the
    irrigation system will be our responsibility as owner and must be corrected using only the Bonita
    Lakes Approved Irrigation Vendor.
    I/We understand and acknowledge that by the above Opt-Out we understand we must ensure
    that the residence listed above is cleared of all debris within 30 days of the hurricane date to
    allow the normal landscape activities to resume (unless we have Opted Out of all association
    landscape contracted work). Should this not be completed, and regular contracted landscape
    activities are unable to resume at our residence we can be subject to Non-Compliance Notice per
    the Community Wide Standards.
